Odontogenic rhinosinusitis( OR) was first recognized in the 17th century, when the anatomist and physician Nathaniel Highmore identified an abscess within a canine tooth as the cause of a maxillary sinus infection. Today, however, odontogenic causes of maxillary sinusitis are often overlooked, with the reported incidence of OR ranging widely between 4.6 and 77%. The high variability may be due to a lack of consensus in diagnosing OR. The symptoms and physical exam results of odontogenic and non-odontogenic rhinosinusitis are similar, and only 30% of OR patients present with cheek or tooth pain. Some studies have reported that around 20% of OR patients were only diagnosed after a retrospective review. OR is the rhinosinusitis of dental origin and modern interpretations of OR recognize diverse etiologies including classic( dental extractions, endodontic treatments, etc) , implantologic, or preimplantologic( maxillary sinus elevation, Le Fort osteotomies) . Management options for OR are similar to that of non-odontogenic sinusitis, with the addition of dental surgery complementing medical management and endoscopic sinus surgery. Concomitant management of the dental origin and the associated sinus infection will ensure complete resolution of the disease and may prevent recurrence.
